New Practice Member Paperwork

After scheduling your initial visit to Revival Chiropractic, our team will email specific paperwork tailored to your individual health goals. We ask that you provide as much detail in your paperwork about your specific needs. This will help Dr. Brad connect with who you are and help us serve you. You must have the paperwork filled out before your first visit to the office or your appointment may be rescheduled.

Health History & Complete Consultation

A one-on-one consultation will take place to discuss what health challenges you’re currently experiencing, review your history so we may better understand what has led you to your current health status, and determine your goals for the future.

Neurological Evaluation & Spinal Exam

Using our state of the art technology, we assess your entire spine and nervous system to get an exact assessment of your problem areas and any existing nerve interference. Our goal is to discover the root cause of any disease or dysfunction in your body.

Digital Chiropractic Postural X-rays

If necessary, using our state-of-the-art, digital x-ray system, we are able to identify any curves, degeneration or loss of disc space in your spine, which cannot be detected during the other evaluations.

First Chiropractic Adjustment

We utilize the most researched technique in the profession. It is gentle, specific, and effective. The adjustment involves no twisting or cracking and we’ve seen kids, grandparents, and everyone in between love our adjustments!

Individualized Report of Findings

Following the initial appointment, Dr. Brad will prepare a report of findings where we will go over our exam findings, the results of your neurological assessments and X-rays, and then provide you and your family with tailored recommendations to help you achieve your health goals. We want to do whatever we can do make care in our office convenient and affordable.
